Comprised of nurse practitioners
and certified psychiatric registered nurses who
visit Residential Care Facilities, Skilled Nursing
Facilities, clients' homes and other outpatient
settings to evaluate, manage and treat individuals
who require psychiatric support. The team is an
adjunct of the psychiatrists' visits. This unique
approach is often able to provide client support
and treatment in the least restrictive care environment
while focusing on the management of psychiatric
symptoms. The Nurse Practitioner Team provides mental
status evaluations, medication management and antipsychotic
injections, crisis management and triage, supportive
psychotherapy, and care coordination with other
psychiatric support services.
